The mental health screening form is a brief screening tool that can alert clinicians or clinical staff to the possibility of the existence of mental health problems. It was originally designed for use in substance abuse treatment programs to screen for mental health problems that might coexist with mental health problems. It is not intended as a diagnostic instrument.
