Neuropsychiatric Inventory Questionnaire (NPI-Q)
What It Measures
The Neuropsychiatric Inventory Questionnaire (NPI-Q) is a short form filled out by a family member or other informant about someone living with dementia. It covers 12 kinds of behavior change: delusions, hallucinations, agitation or aggression, depression, anxiety, elation, apathy, disinhibition, irritability, repetitive motor behavior, nighttime behavior, and changes in appetite or eating.
For each one the informant first answers yes or no. If yes, they rate how severe it has been over the past month on a 1-to-3 scale, and separately how much distress it causes them on a 0-to-5 scale. That produces two totals: severity out of 36 and distress out of 60.
