What It Measures
The 12-Short Form Health Survey (SF-12) is a short self-report tool. It measures overall health and daily well-being. The survey asks twelve simple questions. It shows how health affects a person's everyday life. It checks physical health and mental well-being. Occupational therapy, physical therapy, and mental health clinicians use these scores. They help measure quality of life, track progress, and plan daily care.
Health problems can limit daily activities, work tasks, and social life. For this reason, the SF-12 is useful in many settings. These include hospitals, rehab centers, outpatient clinics, mental health units, nursing facilities, home health care, community programs, and telehealth.
Clinicians use this survey with adults and seniors. It is often used with individuals recovering from a stroke. The scores help track health changes over time, guide care decisions, and set recovery goals.
