World Health Organization Assessment Schedule 2.0 (WHODAS 2.0)
What It Measures
WHODAS 2.0 is a general-purpose questionnaire that measures how much a physical or mental health condition is affecting a person's everyday functioning. It produces scores across six domains: cognition, mobility, self-care, getting along with others, life activities, and participation, and can be completed by the person themselves, by an interviewer, or by a caregiver.
Because it isn't tied to any single diagnosis, WHODAS 2.0 works across a wide range of conditions and care settings, including telehealth. It comes in a 36-item and a shorter 12-item version, with domain-specific and overall disability scores that support consistent tracking over time. Occupational, physical, and mental health therapists use it to document baseline functioning and monitor change across acute, inpatient, outpatient, and community settings
