Activity Measure for Post-Acute Care

The Activity Measure for Post-Acute Care (AM-PAC) tracks how much of daily life a person can manage on their own while moving through the hospital and into rehab, home health, or outpatient care. It covers three areas: basic mobility (rolling in bed, transfers, walking, stairs), daily activity (dressing, bathing, toileting, eating), and applied cognition (comprehension and memory).

Most teams use the inpatient short forms, known as "6-Clicks," which pull six questions from each domain and score them 1 to 4 for a domain total of 6 to 24. Answers can come from the patient or from a proxy; in hospitals the treating therapist usually scores them from observation. Because the same metric follows the patient across settings, teams use it to prioritize who needs therapy and to inform discharge planning.
