6 items assessing of motor (eating, grooming, bowel management and toilet transfers) and cognitive (expression and memory) function, which can be reliably collected in acute care. For patients who are able to walk 150 feet or more, eating and grooming items are replaced by items evaluating walking and bed transfer.

License is required. To be credentialed, health care professionals must complete the mandatory e-Learning module and pass a credentialing test with a mark of over 80%. Assessors are required to credential every two years.
