Developmental Profile 4

The Developmental Profile 4 (DP-4) is a questionnaire completed by someone who knows a child well, describing what that child can already do. It covers five areas of development: physical, adaptive behavior, social-emotional, cognitive, and communication — the same five domains IDEA specifies for assessing developmental delay.

Items use a plain yes-or-no format, each describing one specific skill, which is what keeps the assessment quick despite covering birth through age 21. Standard scores compare a child with peers their own age. Growth scores are the more useful output for children functioning at a very low level, where standard scores bottom out and cannot show that anything has changed. Every item comes with a matched intervention activity.
