
If you own a rehab therapy clinic, choosing the right EMR/EHR system is one of your most important tasks.
If you’re an OT, PT, or SLP preparing for a job interview. one essential question to ask is: Which electronic health record (EHR) or electronic medical record (EMR) system does the organization use?
Why?
Because the technology driving their practice will also drive yours—and that impacts your therapy, your efficiency, and your patients’ journey.
The EMR isn’t just a behind-the-scenes detail; it shapes how smoothly evaluations flow, how quickly you can document, how effectively you can track progress, and even how connected your patients feel to their care.
Asking about it during your interview isn’t just smart—it’s essential for understanding how your clinical expertise will translate into practice in that setting.
As we head into 2025, we are going to see massive changes in the capabilities of our EMR systems. This is why I wanted to compare the different options on the market to help you find the right EMR for you!
A quick note on language: You can see we are using EMR/EHR interchangeably. An electronic health record is technically more expansive than an electronic medical record and best reflects most of the software below, but we’ve found that most clinicians still utilize the term EMR.
Changing expectations in 2025
If the EMR you are demoing still looks like a 1990s tax document, I would honestly run for the hills.
The early versions of EMRs created a huge level of headache/burnout without the promised benefits. But, new technology is rapidly changing what we should expect from our EMRs. Your PT/SLP/OT documentation should should function as a powerful tool that enhances your care—not a constant obstacle contributing to frustration and burnout.
Your ideal EMR should be leveraging AI, both as a scribe and an integrated support system within the platform itself. It also should connect with your billing and scheduling software, creating a fully cohesive workflow that saves time, reduces errors, and lets you focus on what matters most: your patients.
Choosing the right therapy EMR for you
Here are some of the top EMRs that we found that were built for outpatient occupational therapy, physical therapy, and speech therapy. We are leaving out the big players in the hospital systems (like CERNER) as they do not cater to private therapy practices.
We only include EMRs and EHRs that meet these non-negotiable criteria: they must be HIPAA compliant, offer built-in telehealth functionality, be fully cloud-based, and specifically designed to support the unique needs of OT, PT, and SLP.

A quick note on Google Workspace
I do know of occupational therapy private pay therapy practices that use Google workspace in lieu of an EMR.
To do this you would need to upgrade to the HIPPA compliant version of Google Workspace. You’ll also need to have a clear understanding of the data you need to capture—and a solid plan for organizing, tracking, and using it effectively.

I think simple practice is widely used. Is there a reason why it was not included in the list?
Hi Fatma! Great question! We have a 2nd batch of EMRs that we are adding to this list this week! I just made sure we had Simple Practice included! (Honestly there were so many options that we decided to put out a round one- just to make sure they way we were organizing the info is helpful, and then plan to update it in about a week with more options!!)

Thank you so much for this resource Sarah! I am systematically going through the chart and looking into each company for my private practice and getting demos for the ones that I’m interested in. It is so helpful just to have a jumping off point because there are so many out there and it can be overwhelming, especially when the definitions of certain features are not always consistent among different companies. I like the simplicity and features offered on Theraplatform, but I wish they had an option for a built-in AI scribe. Simple practice is one that a lot of mental health base practitioners use-I wasn’t that impressed with it for my needs and it seemed overly cluttered with a lot of features and forms that I wouldn’t use. It is great to hear recommendations as well as a place to compare features. It sounds like it is coming but I hope to see more companies integrate AI scribes within the EMR System as an option. Thanks for providing this resource and others about AI, they have been so valuable!

You mentioned Google Workspace too–curious of your opinion (or others) of starting a practice and then switching as it grows (ie Google Workspace to start–>EHR platform later)… I want to be as streamlined and efficient with processes and structure as possible from the get go and minimize some of the “busy work” involved if feasible to do so. Thank you! I’m considering Healthie, which I don’t see on this list. It is free if you have under 10 clients and I’ve heard is easy to switch to another EMR if you decide to. I heard of someone with Therapy Notes who kept paying for that plus another because they couldn’t figure out how else to save their data. I have heard Healthie isn’t great if you decide to use insurance.
I’ve tried a few of the softwares, but PromptEMR has worked really well for us in our expansion efforts into OT

Thank you for creating and updating this chart. I wish I had this resource when I started my practice 3 years ago. I want to tell you my experience with Simple Practice to give you more data for your analysis. The focus of my solo practice is on helping families with sensory-based issues, with special emphases on sleeping, eating, and self-regulation. I work with families of toddlers to young adults. Simple Practice is the EHR I chose, because it seemed more…simple. My notes serve as learning opportunities for my families, so blank SOAP boxes are fine for me. SP is integrating AI into their teletherapy platform, but for a fee. I don’t do enough teletherapy to warrant it. I created my own evaluation, treatment plan, and forms on the site (with some help because I am a slow-tech 65-year old). I do wish I could just upload templates for specific things like the SCOPE forms or the BOT , but I developed a workaround that works nominally well for me.I use the calendar to keep track of clients. They get auto-reminders of their appointments. Simple practice does the billing, clients pay online, I never have to do a transaction at the time of a client session, which I like. The client portal is linked to my website. I’ve thought of switching to Jane because on the face of it it seems less expensive for the features I want, but as I noted, I am very slow-tech and don’t have the time right now to analysethat. I hope that this little slice of real-life use by a solo practitioner is helpful. I love being a member of the OTPotential community and I thank you again for all you are doing for our profession!
